Here’s what you can expect during your first visit with us.
We’re very happy that you’ve chosen us for Dental Care needs! We aim to make each and every experience with us positive, and though we know that the dentist is not everyone’s favorite place to be, we do try and make your visit with us as comfortable and pain free as possible.
We strive to be a well rounded patient-centered practice. We believe in treating our patients as equal partners in planning, developing and monitoring care with a high level of professionalism and respect.
We have invested time and resources into providing the latest technologies in Dentistry to our patients so that we may serve you with as many treatment options as possible, eliminating the need for multiple dental appointments or visits with a specialist.
During your initial appointment with us, we will spend time getting to know you and addressing your dental concerns. Please be prepared to share your current concerns and even your past dental experiences with us. In order to establish records, we will:
Do a Comprehensive Medical History Review, please bring a list of medications you are currently taking
Establish a baseline Blood Pressure Check
Take radiographic records including Bitewings and a Panoramic radiograph which gives us a 3D view of your teeth!
Scan your teeth with iTero scanning technology, giving us a digital model of your teeth, allowing you to see what we see.
Record Periodontal Probing depths to confirm gingival tissue health
Chart existing teeth and restoration you currently have
Perform a thorough Oral cancer screening
Assess you for grinding and clenching habits and possible intervention needs
Evaluate you for Orthodontic needs
Take intra-oral photo’s of existing restorations
Establish a base line Head, Neck and Oral facial muscle examination.

New Patient Forms
If you would like to print and fill out our new patient forms prior to your appointment, please click on the link boxes below to access our new patient forms. If you want to complete our forms in office, please arrive 15 minutes prior to your appointment.
If you are an existing patient who would like records released to another office, please print the completed records release form, bring it to our office, or email it to us at info@fairbanksdentalcenter.com and we will be happy to assist in transferring records.
If you are coming to us from another office, you may need to contact your previous Dentist to request your records and sign their release. We can not get records from a previous office if you haven’t signed for a release of said records.
About Insurance
Our office is currently contracted with the following dental insurance carriers:
Delta Dental Premier
Premier Blue Cross of Washington and Alaska
Met Life
Our office can still bill non-contracted insurance carriers, however you may have a larger out of pocket financial obligation with us.
We are not able to bill Medicaid, Medicare, Tricare or Denali Kid Care.
